Primary Medical Care Physician Survey

The Michigan Department of Community Health is attempting to analyze the availability of primary medical healthcare in your area. More specifically, they are trying to determine whether there is a shortage of primary care physicians as compared to the total population. In order to do this, they need you to fill out this survey (2 pages) for each primary care physician you have at your offices. Primary Care Physicians include those who specialize in General Practice, Family Practice, Pediatrics, Internal Medicine and Ob/Gyn.
