NOTE: Michigan Medicaid follows this guideline, but with a few differences. They are:
1. Distant and originating sites must be at least 50 miles apart2. MI Medicaid covers more originating sites including county mental health clinic or publicly funded mental health facility,Renal dialysis facility (not covered in Medicare list). skilled nursing facility, Tribal Health Center (THC),
3. Tthe services of clinical psychologists and social workers cannot be billed directly - they must be billedthrough a specified facility.
